Bring your original COVID-19 vaccination card/vaccine record and proof of insurance (if applicable) with you to your appointment.

Moderna and Pfizer COVID-19 Vaccine booster dose can be given at least 6 months after completion of the primary series to individuals:

• 65 years of age and older
  • 18 through 64 years of age at high risk of severe COVID-19
  • 18 through 64 years of age with frequent institutional or occupational exposure to SARS-CoV-2

Janssen (Johnson and Johnson) COVID-19 Vaccine booster can be given 2 months after completion of the single-dose primary regimen to individuals 18 years of age and older.

Per CDC guidelines, patients can now “mix and match” COVID-19 vaccines. If you would like to receive a different brand than what you received as your primary series, please schedule on the day of the desired vaccine.
